Next-Generation Neuropharmacology of Anesthetics and Analgesics: From Molecular Targets to Precision Safety
The pharmacology of anesthetics and analgesics is evolving rapidly, driven by a deeper understanding of the molecular and neural mechanisms that govern pain and consciousness. We are moving away from a "one-size-fits-all" approach to opioids and volatiles, and toward a mechanistic understanding of how these powerful drugs interact with neural circuits, the blood-brain barrier, and the immune system.Current challenges are twofold: (1) The need for novel targets to treat neuropathic pain without addiction (e.g., Nav blockers, biased opioid agonists, ketamine); and (2) The need to minimize neurotoxicity and optimize delivery of existing anesthetics (e.g., TCI modeling, BBB transport). This Research Topic aims to bridge the gap between molecular discovery (receptor binding, ion channels) and clinical pharmacology (pharmacokinetics, toxicity, and dosing strategies). We seek to understand not just how these drugs kill pain or induce unconsciousness, but how we can manipulate these pathways for safer, more effective outcomes. We welcome Original Research, Reviews, and Mechanistic Studies focusing on:1. Novel Molecular Targets: Allosteric modulation of Opioid, GABA, and NMDA receptors (e.g., Ketamine mechanisms). Targeting Ion Channels (Nav1.7/1.8, TRP channels) and GPCRs for non-opioid analgesia.2. Neuroinflammation & Tolerance: The role of microglia, cytokines, and astrocytes in analgesic tolerance and hyperalgesia. Mechanisms of transition from acute to chronic pain.3. Pharmacokinetics & Delivery: Transport across the Blood-Brain Barrier (BBB). Target-Controlled Infusion (TCI) models and PK/PD relationships in anesthesia. Novel delivery systems (intrathecal, nanotechnology).4. Safety & Neurotoxicity: Mechanisms of anesthetic-induced neurotoxicity and apoptosis (developmental and geriatric).
