Novel biomarkers and precision from pathogenesis to targeted therapies in systemic vasculitis

Systemic vasculitis comprises a heterogeneous group of immune-mediated disorders characterized by inflammation of blood vessels, often leading to irreversible organ damage and substantial morbidity. Despite significant advances in our understanding of disease pathogenesis and the introduction of targeted therapies, important challenges remain in routine clinical practice. Early diagnosis is frequently delayed, reliable biomarkers are unavailable for many clinical settings, and accurately predicting disease activity, relapse, or therapeutic response continues to represent a major unmet clinical need.Recent advances in immunology, molecular medicine, and high-throughput technologies have transformed the landscape of vasculitis research. Multi-omics approaches, single-cell technologies, advanced imaging, and artificial intelligence are providing unprecedented opportunities to unravel disease mechanisms and identify biomarkers with genuine clinical utility. These developments are driving a paradigm shift towards precision medicine, where diagnosis, risk stratification, and therapeutic decisions are increasingly tailored to the biological characteristics of individual patients rather than relying solely on conventional disease classifications. Nevertheless, knowledge remains fragmented across different vasculitis subtypes, highlighting the need for an interdisciplinary platform that integrates biomarker discovery with translational and clinical research. The aim of this Research Topic is to bring together basic scientists, translational researchers, and clinicians to present the latest advances in systemic vasculitis, with a particular focus on novel biomarkers and precision medicine. By encouraging contributions that span experimental research, translational investigations, and clinical studies, this collection seeks to bridge the gap between mechanistic discoveries and patient care, ultimately facilitating the implementation of more personalized diagnostic and therapeutic strategies. We welcome Original Research Articles, Clinical Trials, Reviews, Systematic Reviews, Meta-Analyses, and Brief Research Reports covering basic, translational, and clinical research, including experimental and observational studies, that address all aspects of systemic vasculitis. Topics of interest include, but are not limited to:• Novel biomarkers for diagnosis, prognosis, disease monitoring, treatment response, and relapse prediction • Immunopathogenesis and molecular mechanisms of vasculitis• Precision diagnostics and biomarker-guided patient stratification • Multi-omics technologies, including genomics, transcriptomics, proteomics, metabolomics, epigenomics, and single-cell approaches • Targeted biological therapies, small-molecule inhibitors, and emerging therapeutic strategies • Artificial intelligence and digital health applications in vasculitis research and clinical decision support • Translational research, clinical trials, real-world evidence, and personalized management strategies across the spectrum of systemic vasculitis, including large-, medium-, small-, and variable-vessel vasculitis Ultimately, this Research Topic aims to provide a comprehensive overview of current advances while identifying future research priorities that will accelerate the translation of biomarker discovery into precision medicine. By fostering collaboration between researchers and clinicians across disciplines, we hope this collection will help improve diagnosis, therapeutic decision-making, and long-term outcomes for patients with systemic vasculitis.
