Omics-Based Biomarkers in Renal Endocrinology
The rapid expansion of omics technologies has transformed our ability to characterize molecular pathways underlying kidney disease. In renal endocrinology, these approaches offer unprecedented opportunities to identify biomarkers that not only reflect structural injury, but also capture hormonal regulation, metabolic signaling, and inter-organ endocrine cross-talk. However, translating omics discoveries into clinically meaningful tools for risk stratification, disease progression, and therapeutic decision-making remains a major challenge. This Research Topic aims to highlight advances in genomic, transcriptomic, epigenomic, proteomic, metabolomic, and multi-omic approaches that drive biomarker discovery and validation within renal endocrinology. We particularly welcome contributions addressing genetic and molecular biomarkers in diabetic kidney disease (DKD), given its central role as a leading cause of chronic kidney disease worldwide. Studies exploring biomarkers that predict DKD onset, progression (e.g., eGFR decline, albuminuria trajectory), therapeutic response, or cardiovascular–renal risk are strongly encouraged.Beyond DKD, this collection seeks work that situates biomarker discovery within the broader endocrine context of kidney disease, including pathways related to vitamin D metabolism, FGF23/klotho signaling, RAAS activation, erythropoietin regulation, mineral metabolism, mitochondrial dysfunction, and oxidative stress. We welcome mechanistic studies linking omics findings to endocrine regulation, as well as clinical and translational research evaluating biomarker performance in well-characterized patient cohorts.Topics of interest include, but are not limited to:• Identification and validation of genetic variants, epigenetic signatures, and transcriptomic profiles associated with renal endocrine dysfunction• Proteomic, metabolomic, and extracellular vesicle–based biomarkers reflecting hormonal and metabolic pathways in kidney disease• Multi-omic integration for improved risk prediction and patient stratification• Biomarkers of therapeutic response in endocrine-modulating interventions (e.g., RAAS inhibitors, mineral metabolism therapies)• Causal inference approaches, including Mendelian randomization, when appropriately validated• Standardization, reproducibility, and clinical implementation of omics-based biomarkersBy bringing together mechanistic and clinical perspectives, this Research Topic aims to advance the development of robust, biologically grounded biomarkers that improve understanding of endocrine regulation in kidney disease and ultimately support precision medicine in renal endocrinology.
