Pediatric Autoimmune Liver Disorders: Current Trends and Innovations

Autoimmune liver disorders in children are a rapidly evolving area within pediatric hepatology, reflecting significant progress in the understanding, diagnosis, and management of conditions such as autoimmune hepatitis, autoimmune sclerosing cholangitis and de-novo post-transplant autoimmune hepatitis. Despite advancements, these disorders remain rare and complex, with multifactorial etiologies linked to immune dysregulation, genetic susceptibility, and unique pediatric features. Recent studies have begun to illuminate the intricate pathogenic mechanisms—ranging from T-cell-mediated liver injury to microbiome alterations—and how these insights can inform novel diagnostic and treatment strategies. However, gaps persist in early detection, reliable biomarker development, therapy selection, and prediction of disease progression, exacerbating the challenge of personalized care and sometimes resulting in severe complications, such as cirrhosis and the need for transplantation. This Research Topic aims to bring together the latest evidence and multidisciplinary perspectives on pediatric autoimmune liver disorders. We seek to address key questions related to immune pathogenesis, innovation in diagnostics (including imaging and serology), and the translation of emerging molecular and clinical insights into targeted therapies for children. Our goal is to stimulate a comprehensive discussion that bridges mechanistic science and clinical realities, clarifies controversies on management protocols, and identifies best practices for early diagnosis, treatment optimization, and long-term outcomes. Contributions elucidating how emerging technologies such as precision medicine and multi-omics can be integrated into care pathways for pediatric populations are particularly welcome.Given the multifaceted nature of these diseases, this Research Topic encompasses pathogenesis, clinical challenges, and state-of-the-art solutions in pediatric autoimmune hepatology, with an emphasis on both innovation and practical management. We are interested in original research, reviews, perspectives, and case reports, particularly those that connect mechanistic discoveries to bedside decision-making. To gather further insights into this field, we welcome articles addressing, but not limited to, the following themes:•Pathogenesis and immune mechanisms in pediatric autoimmune liver disorders•Advances in biomarkers and diagnostic strategies•Novel insights into clinical management and therapy optimization•Outcomes, complications, and transplantation in pediatric settings•Integration of precision medicine and omics approaches•Quality of life and long-term follow-up•Clinical trial updates and emerging therapeutic modalities•The role of the microbiome and genetics in disease progression•Transition from pediatric to adult services in pediatric autoimmune liver disordersReviews, mini-reviews, original research, perspectives, brief research reports, and case studies are all welcome as part of this Research Topic.
