Personalized strategies for managing emergent toxicities in advanced solid tumor therapies

Advances in systemic treatments for advanced solid tumors have significantly improved survival, yet they have also introduced a diverse and increasingly complex spectrum of treatment related toxicities. Immune checkpoint inhibitors, targeted therapies, antibody–drug conjugates, and cellular immunotherapies can trigger adverse events that differ markedly from those associated with traditional chemotherapy, possibly involving every organ or tissue. These toxicities vary widely among patients due to differences in tumor biology, immune status, comorbidities, pharmacogenomic profiles, and prior therapeutic exposures. As treatment regimens become more personalized and multimodal, conventional toxicity management strategies are no longer adequate. There is a growing need for individualized approaches that integrate clinical assessment with molecular, immunologic, and radiologic markers to better predict, detect, and mitigate emergent toxicities while maintaining therapeutic benefit. This Research Topic aims to highlight innovative, patient tailored strategies that can enhance safety, optimize outcomes, and support precision oncology in the era of complex cancer therapies.Emergent toxicities from modern systemic therapies for advanced solid tumors are increasingly heterogeneous and difficult to predict, challenging traditional, uniform management approaches. Treatment-related adverse events vary widely across patients due to differences in immune status, comorbidities, and treatment history. To address this, the field must adopt personalized toxicity management strategies that integrate clinical, molecular, and imaging data. Recent advances, including multi omics biomarkers, immune monitoring, radiomics, machine learning prediction models, and digital health tools for real time symptom tracking, offer new opportunities to anticipate, detect, and mitigate toxicities while preserving therapeutic efficacy and ensuring safer, more individualized cancer care. This Research Topic focuses on advancing personalized approaches for predicting, detecting, and managing emergent toxicities associated with modern systemic therapies for advanced solid tumors. As treatments such as immune checkpoint inhibitors, targeted therapies, antibody-drug conjugates, and cellular immunotherapies become increasingly complex, patients experience highly variable immune‑mediated, metabolic, and organ‑specific adverse events. Traditional, uniform management strategies are no longer sufficient to address this heterogeneity. The Topic aims to highlight innovative, data‑driven frameworks capable of tailoring toxicity management to individual patient characteristics.Contributors are invited to address themes including: biomarkers and multi‑omics signatures predicting toxicity risk; mechanistic insights into treatment‑related adverse events; clinical or computational risk‑stratification models; radiomics and imaging‑based monitoring; machine‑learning and digital decision‑support tools; patient‑reported outcomes and remote monitoring technologies; personalized strategies for multimodal regimens; and approaches for vulnerable populations. Submissions may span translational research, clinical studies, and implementation frameworks supporting precision toxicity management. Please note that manuscripts consisting solely of bioinformatics or computational analysis of public genomic or transcriptomic databases which are not accompanied by robust and relevant validation (clinical cohort or biological validation in vitro or in vivo) are out of scope for this Research Topic.
