Personalized Therapeutic Strategies for Acute Myeloid Leukemia in Elderly Patients

Elderly patients diagnosed with acute myeloid leukemia (AML) represent a distinct and increasingly prevalent clinical population, often characterized by heterogeneous disease biology, higher comorbidity burden, and varying degrees of physiological reserve. The growing incidence of AML in aging populations highlights the need for treatment paradigms tailored to older adults, who may have limited tolerance for intensive chemotherapy. Recent advances in genomics, patient risk stratification, and the integration of geriatric assessments have significantly informed the clinical decision-making process. The introduction of hypomethylating agents, novel targeted therapies, and optimized supportive care strategies has begun to improve outcomes for some elderly individuals, although therapeutic resistance, management of multiple comorbidities, and maintaining quality of life remain central challenges.Despite progress, survival outcomes for elderly AML patients remain suboptimal when compared to younger cohorts, underscoring the ongoing challenge of balancing treatment efficacy and safety within the context of frailty and multimorbidity. Current research and debates focus on refining patient selection through integrated molecular and geriatric assessments, including systematic frailty and fitness evaluations, as well as translating clinical trial findings to real-world healthcare settings. There is a growing recognition that personalized care—incorporating genomics, geriatric risk scores, and patient preferences—is critical to improving prognoses and enhancing daily functioning and survival in this vulnerable population. This Research Topic will explore comprehensive care for elderly AML patients, emphasizing translational, clinical, and patient-oriented research designed to optimize outcomes in this group. We welcome only Original Research, Review, Mini Review, and Perspective articles on themes including, but not limited to:o Application and real-world outcomes of hypomethylating agents and new targeted therapieso Role and validation of fitness, frailty, and geriatric assessment tools in the treatment decision processo Prognostic impact of molecular and cytogenetic risk features in elderly AML patientso Strategies for integrating comorbidity management and supportive care to improve overall outcomeso Quality of life measures, patient-reported experiences, and shared decision-making modelso Translating clinical trial advances into routine clinical practice for older adultsPlease note: manuscripts consisting solely of bioinformatics or computational analysis of sole public databases which are not accompanied by validation (independent cohort or biological validation in vitro or in vivo) are out of scope for this section and will not be accepted as part of this Research Topic.
