Personalizing Cancer Immunotherapy Through Pharmacokinetics and Biomarker-Guided Strategies

While immune checkpoint inhibitors and other immunotherapies have revolutionised oncology, therapeutic responses remain highly variable. Most of these agents are administered at standard flat doses, which ignores significant inter-patient pharmacokinetic (PK) variability. Factors such as patient body composition, gender, target-mediated drug disposition (TMDD), and immunogenicity can lead to wide fluctuations in drug exposure. This variability directly impacts treatment efficacy and increases the risk of severe immune-related adverse events (irAEs).To optimize these treatments, clinical practice must bridge the gap between immunology and clinical pharmacology. This collection focuses on integrating therapeutic drug monitoring (TDM), pharmacokinetic/pharmacodynamic (PK/PD) modeling, and host pharmacogenomics with traditional biological biomarkers. By understanding the exposure-response relationship in individual patients, we can move away from one-size-fits-all dosing toward personalized, biologically guided administration strategies that improve clinical outcomes. This Research Topic brings together clinical pharmacologists, immunologists, and oncology researchers to explore the therapeutic optimization of cancer immunotherapies. We welcome original research, clinical trials, reviews, and methodological studies on:* Therapeutic drug monitoring (TDM) of monoclonal antibodies, including anti-PD-1, anti-PD-L1, and anti-CTLA-4 therapies, and novel immunotherapeutics* Pharmacokinetic and pharmacodynamic (PK/PD) characterisation of inter-patient exposure variability and target-mediated drug disposition* Pharmacogenomics, host immunogenetics, and genetic polymorphisms influencing drug clearance, treatment efficacy, and toxicity* Circulating immune biomarkers, soluble checkpoints, and real-time pharmacokinetic monitoring to guide treatment duration and dosing intervals* Profiling exposure–response relationships to predict, monitor, and manage immune-related adverse events (irAEs)* Clinical implementation, feasibility, and cost–benefit analyses of personalised dosing regimens, including weight-based versus flat dosing and extended dosing intervalsPlease note: Manuscripts consisting solely of bioinformatics, computational analysis, or predictions of public databases which are not accompanied by validation (independent clinical or patient cohort, or biological validation in vitro or in vivo, which are not based on public databases) are not suitable for publication in this journal.
