Precision Approaches to Acute Heart Failure: Phenotyping, Risk Stratification, and Early Management.
Acute heart failure is a major cause of emergency cardiovascular admission and remains associated with high short term mortality, frequent rehospitalization, and substantial healthcare burden. Although current management relies on established clinical and hemodynamic principles, acute heart failure represents a highly heterogeneous syndrome shaped by differences in etiology, congestion profile, ventricular function, comorbidities, renal response, and treatment trajectory. This heterogeneity helps explain why uniform treatment pathways often fail to capture individual risk and therapeutic needs. This Research Topic aims to explore how precision medicine can improve the early assessment and management of acute heart failure. Rather than focusing broadly on personalized care across all heart failure settings, this collection will concentrate on the acute episode and the vulnerable period that follows it. We are particularly interested in work that refines clinically meaningful phenotypes, improves risk stratification at presentation, supports tailored decongestive and vasoactive strategies, and links biological or imaging signals with actionable bedside decisions. We welcome original research, reviews, systematic reviews, brief reports, methods papers, and clinically focused perspectives on topics including:• Phenotyping and subclassification of acute heart failure at presentation;• biomarkers for diagnosis, congestion, treatment response, and short-term prognosis;• hemodynamic profiling and imaging guided management;• cardiorenal and cardiometabolic interactions in acute decompensation;• tailored strategies for decongestion, vasoactive therapy, escalation of care, and short-term mechanical circulatory support for cardiogenic shock management;• Early post discharge risk, transitional care, and prevention of rehospitalization;• Clinically relevant precision approaches in special populations, including older adults, women, and patients with preserved ejection fraction.
