Precision Medicine and Anti-Inflammatory Therapies in Heart Failure: Phenotypes, Drug Repurposing, and Novel Targets
Inflammation and immune dysregulation are increasingly recognized as important contributors to the development and progression of heart failure. However, their effects vary substantially across patients and heart failure phenotypes, including heart failure with reduced ejection fraction (HFrEF) and preserved ejection fraction (HFpEF). This biological and clinical heterogeneity may help explain the mixed results of previous anti-inflammatory trials and highlights the need for more precise approaches to patient identification and treatment. Advances in biomarker discovery, molecular profiling, imaging, multi-omics, and artificial intelligence now offer new opportunities to characterize inflammatory pathways and identify patients most likely to benefit from immune-targeted interventions. This Research Topic aims to explore the mechanisms linking inflammation and immune dysregulation with heart failure and to assess their implications for diagnosis, risk stratification, and treatment. Particular attention will be given to biomarker-guided patient selection, precision pharmacotherapy, cytokine and inflammasome inhibition, sphingosine-1-phosphate receptor modulation, pharmacogenomics, drug repurposing, and the development of novel targeted therapies. By bringing together basic, translational, and clinical perspectives, the Research Topic will advance understanding of how individualized anti-inflammatory strategies can be incorporated into cardiovascular medicine. We welcome original research, translational and clinical studies, systematic reviews, meta-analyses, and expert perspectives addressing, but not limited to, the following themes:-Inflammatory mechanisms in HFrEF and HFpEF-Immune-cell activation and cardiac remodeling-Cytokine, chemokine, and inflammasome signalingBiomarkers for inflammatory phenotyping and treatment selection-Multi-omics and molecular profiling in heart failure-Imaging of inflammation and cardiac tissue remodeling-Precision pharmacotherapy and biomarker-enriched clinical trials-Cytokine and inflammasome inhibition-Sphingosine-1-phosphate receptor modulation-Drug repurposing and novel immune-modulating therapies-Pharmacogenomics and treatment-response prediction-Artificial intelligence-assisted phenotyping and risk stratification-Translational challenges in developing anti-inflammatory therapies Topic editors: Naseer Ahmed, Alessio Rungatscher, Rahul Mallick Source: https://www.frontiersin.org/research-topics/85197/precision-medicine-and-anti-inflammatory-therapies-in-heart-failure-phenotypes-drug-repurposing-and-novel-targets
