Precision Medicine in Hematologic Malignancies: From Molecular Classification to Personalized Therapy
Hematologic malignancies are increasingly defined by molecular heterogeneity that influences diagnosis, prognosis, and therapy response. Advances in cytogenetics, next-generation sequencing, epigenetics, and single-cell methods have refined disease classification and revealed actionable vulnerabilities across leukemias, lymphomas, and plasma cell disorders. At the same time, targeted therapies, antibody-based approaches, cellular therapies, and response-adapted strategies have expanded treatment options—but they also raise new questions about patient selection, sequencing, and resistance. Precision medicine aims to match the right patient to the right treatment at the right time using integrated clinical and molecular evidence. Despite major progress, many biomarkers remain incompletely validated, and translating complex testing into consistent clinical decisions remains challenging across centers. This Research Topic will focus on how precision medicine can improve outcomes in hematologic malignancies through biomarker-driven diagnosis, risk stratification, and individualized treatment strategies. The collection will emphasize studies that connect molecular findings to actionable clinical endpoints. We welcome contributions that address implementation in real-world practice: selecting and interpreting assays, linking biomarkers to therapy choice and monitoring, and managing resistance or toxicity with evidence-based personalization. We especially encourage manuscripts that provide robust validation, demonstrate clinical utility, or propose practical frameworks for integrating precision testing into modern care pathways. By bringing together European and American perspectives, this Research Topic aims to clarify emerging standards and highlight approaches that can be adopted across diverse healthcare systems. We welcome Original Research, Review, Mini Review and Perspective articles on themes including, but not limited to:- Molecular and cytogenetic determinants of risk across HM, including integrated models aligned with current classification systems (WHO/ICC)- Predictive biomarkers for targeted therapy, including TKIs, BCL2 inhibitors, FLT3/IDH inhibitors, BTK inhibitors, and antibody-based therapies- Precision approaches in chronic leukemias and myeloid neoplasms, including response milestones, treatment-free remission strategies, and transformation risk- MRD-guided treatment adaptation, including assay selection, thresholds, and clinical trial or real-world evidence- Mechanisms of resistance and clonal evolution, and how these inform therapy sequencing or combination strategies- Clinical implementation: testing algorithms, quality assurance, turnaround time, and decision-support frameworksPlease note: manuscripts consisting solely of bioinformatics or computational analysis of sole public databases which are not accompanied by validation (independent cohort or biological validation in vitro or in vivo) are out of scope for this section and will not be accepted as part of this Research Topic.
