Precision Pharmacology of Small-Molecule Targeted Therapies in Hematology: From PK/PD Modeling to AI-Driven Clinical Decision Support
Precision pharmacology in hematology and hemato-oncology is a rapidly advancing field, driven by the emergence of small molecule targeted therapies and transformative innovations in quantitative pharmacology and artificial intelligence (AI). The evolving landscape of hematology—spanning both benign blood disorders and hematological malignancies—necessitates an integrated approach to optimize therapeutic outcomes and safety. With increasingly complex pharmacokinetics, narrow therapeutic windows, and frequent drug–drug interactions, especially in patients exposed to polypharmacy, precise dose individualization and real-time drug monitoring have become pivotal in clinical management. Growing evidence demonstrates that model-informed approaches, advanced drug exposure-response modeling, and digital health tools can enhance efficacy while minimizing toxicity, but their implementation in routine hematology practice remains variable. This Research Topic aims to advance the understanding and integration of precision pharmacology for patients with hematological diseases and blood cancers, focusing on the application of small molecule targeted agents. Specifically, it seeks to collect cutting-edge original research, comprehensive reviews, and clinical perspectives that illustrate innovations in quantitative pharmacology—such as population and physiologically based pharmacokinetics (PopPK/PBPK), pharmacokinetic-pharmacodynamic (PK/PD) modeling, and therapeutic drug monitoring (TDM)—and explore the impact of drug–drug interactions and AI-driven decision tools. Through multidisciplinary contributions, this collection will address unanswered questions regarding individualization of therapy, bioanalytical strategies, real-world implementation, and the integration of AI for predicting drug exposure, optimizing dosing, and preventing adverse drug events in the hematology and hemato-oncology domains. The scope of this Research Topic encompasses all aspects of quantitative and precision pharmacology as they relate to hematology and hematological malignancies, with a particular focus on small molecule targeted drugs.
