Prognostic Biomarkers and Personalized Therapy Approaches in Chronic Myelomonocytic Leukemia
Chronic myelomonocytic leukemia (CMML) is a complex hematologic malignancy that straddles the boundaries between myeloproliferative neoplasms (MPN) and myelodysplastic syndromes (MDS), with a propensity to transform into acute myeloid leukemia (AML). A remarkable feature of CMML is the prevalence of genetic mutations, noted in over 90% of cases, chiefly involving genes like TET2, ASXL1, SRSF2, and the RAS pathways. These mutations influence not only the clinical presentation and progression of CMML but also various prognostic aspects, including the emergence of special CMML variants and precursor phases. Despite the plethora of mutations discovered, the independent prognostic significance of ASXL1 mutations has been consistently validated, securing its role in new prognostic models. Recent studies have propelled the understanding of these mutations, yet several gaps remain, particularly about their mechanistic pathways leading to CMML progression and transformation. This Research Topic aims to further delineate the prognostic significance of genetic mutations in CMML and to integrate these insights into personalized treatment strategies. It endeavors to answer pivotal questions such as how genetic mutations alter disease prognosis and treatment response, and which combinations of mutations might predict clinical outcomes more accurately. Furthermore, it seeks to expand existing prognostic models by incorporating genetic profiling to refine risk stratification and treatment tailoring. An underlying hypothesis is that integrating mutational data with clinical parameters will potentiate more accurate prognostic assessments. To gather further insights into the interrelationships between genetic mutations, prognosis, and therapeutic interventions in CMML, we welcome articles addressing, but not limited to, the following themes:- The role of TET2, ASXL1, and other mutations in CMML pathogenesis and progression.- Clinical implications of mutational combinations on treatment response, especially to hypomethylating agents.- Development and refinement of advanced prognostic models incorporating genomic data.- Personalized treatment approaches informed by genetic profiling in CMML.- Novel therapeutic targets and design of clinical trials addressing specific genetic mutations, particularly within the RAS pathway. Please note the following conflict of interest disclosures for Dr. Nikolaos Papadantonakis - he received research funding support from:- Curis.- Agios Pharmaceuticals.- Novartis.Dr. Ismail Elbaz Younes and Dr. Palomo declared no conflict of interest. Please note that manuscripts consisting solely of bioinformatics or computational analysis of public genomic or transcriptomic databases that are not accompanied by validation (independent cohort or biological validation in vitro or in vivo) are out of the scope of this Research Topic.
