Progression and Long-Term Outcomes of Chronic Kidney Disease in Children: From Biomarkers to Transition of Care
Chronic kidney disease (CKD) in children remains a profound global health challenge with severe implications for long-term health, development, and quality of life. Unlike adult-onset CKD, pediatric CKD is often driven by congenital anomalies of the kidney and urinary tract (CAKUT) or hereditary glomerulopathies, requiring highly specialized management from infancy through adolescence.Predicting disease progression, preventing cardiovascular and metabolic complications, and managing bone mineral disorders are critical to improving pediatric patient outcomes. However, identifying reliable prognostic tools and standardizing care models across different development stages remains difficult. In addition, the transition from pediatric nephrology to adult care is a vulnerable period where treatment non-adherence and graft failure rates rise sharply. This Research Topic aims to compile the latest clinical and translational research on pediatric CKD progression, emphasizing early prediction and continuity of care. We welcome submissions that address diagnostic, therapeutic, and care coordination challenges in pediatric nephrology.Scope and Key ThemesWe welcome submissions of Original Research, Review, Mini-Review, and Clinical Trial articles focusing on, but not limited to, the following areas:• Predictive Biomarkers: Identification and validation of urinary, serum, or genetic biomarkers to predict CKD progression and acute-on-chronic kidney injury.• Cardiovascular and Metabolic Complications: Evolving approaches to monitor and manage arterial stiffness, hypertension, and mineral bone disease in children with CKD.• Neurodevelopment and Quality of Life: Assessing the impact of chronic kidney dysfunction and dialysis modalities on cognitive development, growth, and psychosocial outcomes.• Precision Medicine and Genomics: The role of genetic sequencing in diagnosing rare pediatric renal conditions and predicting treatment response.• Transition of Care: Dedicated care models, structured protocols, and clinical outcomes associated with transitioning adolescent patients from pediatric to adult nephrology services.• Advancements in Anemia of CKD: Evolving definitions of iron deficiency, ESA use, emerging therapies, and associations with growth, neurocognition, and quality of life.• Growth and Nutrition in CKD: Discussing how growth hormone, nutritional optimization, and electrolyte management affect overall health.• Health Disparities and Social Determinants of Health in Pediatric CKD: Understanding how social determinants of health and health disparities affect outcomes such as cardiovascular outcomes, access to transplant, medication adherence, and school performance.
