Real-world advances in inflammatory diseases: from population data to immunological mechanisms and clinical impact

Inflammatory diseases contain a broad spectrum of immune-mediated conditions, including autoimmune, autoinflammatory, allergic, dermatologic, gastrointestinal, and musculoskeletal disorders. These diseases arise from complex interactions between genetic susceptibility, environmental exposures, and dysregulated immune responses, leading to chronic inflammation and progressive tissue damage. Despite major advances in immunology and the development of targeted immunotherapies, substantial gaps remain in understanding how immune mechanisms translate into long-term disease trajectories and treatment outcomes in routine clinical practice.Controlled clinical trials have provided critical insights into immunopathogenesis and therapeutic efficacy; however, trial populations are often highly selected and may not fully represent the diversity of real-world patients. Individuals with inflammatory diseases frequently present with multimorbidity, polypharmacy, and heterogeneous immune phenotypes that influence treatment response and disease progression. Increasing access to large-scale health data sources, including electronic health records, national registries, biobanks, and population-based databases, now provides unprecedented opportunities to investigate immune-mediated diseases across diverse healthcare settings. When integrated with immunological knowledge, these real-world data enable researchers to explore disease heterogeneity, immune-related comorbidities, treatment response patterns, and long-term safety of immunomodulatory therapies at the population level.A major challenge in the field of autoimmune and inflammatory diseases is bridging the gap between mechanistic immunology and clinical decision-making. While advances in molecular and cellular immunology have identified key immune pathways involved in disease pathogenesis, translating these discoveries into effective and sustainable treatment strategies remains complex. Variations in immune phenotypes, treatment adherence, prescribing patterns, and healthcare systems can substantially influence observed outcomes outside controlled trials. Robust real-world investigations are therefore essential to evaluate how immune-targeted therapies perform in routine care and to better understand the long-term consequences of immune dysregulation across patient populations. This Research Topic aims to highlight emerging research that integrates real-world clinical data with advances in immunology to deepen our understanding of autoimmune and inflammatory diseases. Particular emphasis will be placed on studies that connect population-level observations with underlying immune mechanisms, immune biomarkers, and therapeutic targets. By showcasing innovative analytical approaches and large-scale datasets, this collection seeks to provide clinically relevant evidence that reflects real-world patient populations while advancing knowledge of immune-mediated disease biology. Ultimately, this Research Topic aims to promote a more integrated framework in which epidemiologic insights and immunological discoveries jointly inform precision medicine and patient-centered care in inflammatory diseases. We invite submissions that utilize real-world data, immunological investigation, or their integration to generate clinically and mechanistically meaningful insights into inflammatory diseases across medical specialties.
