Recent Advances in Heart Failure and Ventricular Remodeling: Mechanisms, Clinical Trajectories, and Emerging Therapeutic Strategies
Heart failure remains a leading cause of morbidity, mortality, and healthcare burden globally, despite significant advancements in diagnostic and therapeutic strategies. At the center of its pathogenesis is ventricular remodeling, a complex and dynamic process of structural, functional, cellular, and molecular changes in the myocardium triggered by injury or sustained stress. These remodeling processes drive progression from compensated cardiac dysfunction to symptomatic heart failure, with profound effects on clinical presentation, therapeutic response, and long-term outcomes. Recent progress in cardiovascular science, advanced imaging, biomarker development, and clinical research has greatly expanded our understanding of the mechanisms and clinical trajectories underlying ventricular remodeling. This Research Topic seeks to advance the field by highlighting the latest mechanistic, translational, clinical, and epidemiological insights related to heart failure and ventricular remodeling. We aim to address the need for improved integration of pathophysiological discoveries with early detection, refined phenotyping, accurate risk stratification, and innovative therapeutic strategies. By fostering contributions that span research from basic science to clinical practice and across populations, we hope to clarify the drivers of disease progression, better define the heterogeneity among heart failure phenotypes, and support the development of interventions that promote prevention, reverse remodeling, and improved patient outcomes. We welcome original research, reviews, systematic reviews, meta-analyses, and perspectives addressing themes including, but not limited to:• Molecular and cellular mechanisms in ventricular remodeling• Patterns of remodeling in different heart failure phenotypes and cardiomyopathies• Advancements in imaging, biomarkers, and diagnostic or prognostic tools• Clinical outcomes and predictors of disease progression in heart failure• Epidemiological trends and risk factors in the global heart failure population• Pharmacological, device based, lifestyle, and other emerging therapeutic strategies for cardiac remodeling and disease management• Rigorously designed, scientifically sound studies evaluating evidence based complementary or adjunctive approaches, including Traditional Chinese Medicine related interventions, where findings are supported by sound mechanistic rationale and clear clinical relevance to heart failure or ventricular remodeling.
