Redefining the Multiple Myeloma Treatment Continuum: From Frontline Quadruplets to Advanced Immunotherapy Sequencing
The therapeutic landscape of multiple myeloma has experienced an unprecedented evolution, moving from traditional chemotherapy toward highly targeted, multi-agent regimens and cellular immunotherapies. Recent breakthroughs in frontline treatment, most notably the integration of monoclonal antibodies into quadruplet induction regimens, have drastically improved depth of response and progression-free survival across transplant-eligible and transplant-ineligible patients alike. Concurrently, the clinical maturation of chimeric antigen receptor T-cell therapies and bispecific antibodies has reshaped the management of relapsed or refractory disease. As these agents move earlier into the treatment pathway, clinicians and researchers face pressing questions about optimal sequencing, resistance mechanisms, and the biological drivers of treatment response. A comprehensive, cross-disciplinary view of this evolving continuum is needed to translate scientific progress into meaningful clinical benefit.Goal This Research Topic aims to capture the evolving multiple myeloma treatment continuum, from newly diagnosed disease through heavily pretreated relapse, uniting clinical practice with translational insight. We welcome contributions consolidating evidence on frontline quadruplet regimens and how deep molecular responses, including measurable residual disease negativity, translate into durable outcomes. The collection will also examine therapeutic sequencing at first relapse, weighing the role of cellular therapies against antibody-drug conjugates and other emerging agents, as well as advanced strategies from the fourth line onward, including bispecific antibodies and their real world application in heavily pretreated patients. Equally, we welcome translational contributions on soluble biomarker kinetics, immune profiling, and the bone marrow microenvironment, since these factors increasingly guide treatment choice. Together, these perspectives will help define a practical, evidence based roadmap for personalized myeloma care.Scope and Information for Authors This collection welcomes clinical, translational and real world contributions that map the key decision points across the multiple myeloma treatment journey, while remaining broad enough to reflect the diversity of research shaping the field today. We particularly encourage manuscripts that connect clinical outcomes with underlying biological mechanisms. We welcome Original Research, Review, Mini Review and Perspective articles on themes including, but not limited to: • Frontline quadruplet induction regimens and measurable residual disease guided treatment decisions in newly diagnosed multiple myeloma • Therapeutic sequencing at first relapse, including the positioning of cellular therapies relative to antibody-drug conjugates • Advanced bispecific antibody strategies and real world outcomes from the fourth line of therapy onward • Translational biomarkers and bone marrow microenvironment dynamics shaping response and resistance to anti-BCMA and other novel immunotherapiesPlease note: manuscripts consisting solely of bioinformatics or computational analysis of sole public databases which are not accompanied by validation (independent cohort or biological validation in vitro or in vivo) are out of scope for this section and will not be accepted as part of this Research Topic.
