Redo Aortic Valve Interventions: Surgical and Transcatheter Strategies in Complex Valve-in-Valve Scenarios
The field of aortic valve interventions has witnessed significant advancements, particularly in addressing the growing incidence of structural valve degeneration (SVD) and associated complications following bioprosthetic aortic valve replacements. Redo interventions, necessitated by factors such as patient-prosthesis mismatch or paravalvular complications, have increasingly come into focus. Options like redo surgical aortic valve replacement (SAVR) and transcatheter aortic valve replacement (TAVR) via valve-in-valve (ViV) procedures are at the forefront of contemporary treatment strategies, though the optimal approach remains a topic of ongoing debate and exploration. Researchers are investigating the technical and anatomical challenges of these interventions, ranging from constrained annular dimensions and coronary obstruction risks to limited leaflet mobility and compromised hemodynamics. In response to these challenges, procedural refinements have been adopted to improve safety and bolster long-term durability. Techniques such as leaflet modification, coronary protection with chimney stenting, and surgical aortic root enlargement are being employed to mitigate risks and optimize patient outcomes. While these innovations are broadening the therapeutic arsenal available to cardiologists and surgeons, comprehensive clinical data on outcomes from both surgical and transcatheter strategies remains sparse, underscoring the need for continued research in this field. This Research Topic aims to consolidate knowledge and spark further inquiry into redo aortic valve procedures, whether through surgical, transcatheter, or hybrid approaches. To gather further insights into these procedures, we welcome articles addressing, but not limited to, the following themes:o Novel techniques and innovations in redo aortic valve interventionso Decision-making frameworks and patient selection criteriao Comparative long-term outcomes between surgical and transcatheter approacheso Techniques to address anatomical challenges and improve safetyo Innovations shaping the future of complex aortic valve reinterventionThis topic welcomes original research, reviews, case series, and technical reports within the specified themes.
