Respiratory Viral Epidemiology and Clinical Impact in Immunocompromised and High-Risk Pediatric Populations

The field of pediatric infectious diseases has increasingly recognized the substantial vulnerability of immunocompromised and high-risk children to respiratory viral infections. While advances in supportive care and therapeutics have improved survival rates in children with cancer, post-transplantation, or with chronic diseases, respiratory viruses such as respiratory syncytial virus (RSV), influenza, and others continue to cause significant morbidity and mortality in these populations. Recent studies have illuminated the distinct epidemiological patterns, clinical presentations, and complications faced by this group, yet comprehensive data remain limited, particularly in relation to disease burden, regional differences, and viral co-infections.Ongoing challenges include optimizing prevention strategies, improving early detection, and tailoring management to individual risk profiles. There is a pressing need for targeted research exploring both the short- and long-term impact of respiratory viral infections in these vulnerable pediatric cohorts, as well as for innovative solutions to address diagnostic, therapeutic, and prevention gaps. This Research Topic aims to advance the understanding of respiratory viral epidemiology, clinical outcomes, and management in immunocompromised and high-risk pediatric populations. By highlighting recent advances in surveillance, diagnostics, preventive strategies, and emerging therapies, we seek to stimulate further inquiry into the mechanisms underlying severe disease, identify timely interventions, and support the implementation of effective infection control measures. We particularly welcome original research, reviews, and case studies that address gaps in knowledge and contribute to improved patient care, risk stratification, and outcome prediction in these complex groups. This Research Topic encompasses original research and review work on pediatric patients with increased susceptibility to respiratory viral illnesses, focusing on both broad disease trends and patient-specific challenges. To deepen insight in this field, we welcome contributions that address, but are not limited to, the following themes:•Epidemiology and patterns of respiratory viral infections in high-risk pediatric cohorts•Clinical impact, complications, and outcome predictors among immunocompromised children•Diagnostic advancements and challenges, including viral detection, genotyping, and point-of-care testing•Preventive and therapeutic strategies, including vaccination, prophylaxis, and novel treatments•Surveillance programs, infection control interventions, and health care resource utilization•Long-term sequelae and quality of life outcomes in affected childrenWe invite researchers to submit original research articles, systematic or narrative reviews, case series, and brief reports to contribute to this important field.
