Secondary Hypertension - From Mechanisms to Precision Management

Secondary hypertension is a critical field within cardiovascular and systemic disease research, encompassing diverse etiologies that extend beyond essential hypertension. It represents a substantial and often underappreciated segment of hypertensive disorders, with potentially curable causes rooted in endocrine, renal, vascular, and genetic abnormalities. Despite its clinical importance, secondary hypertension remains underdiagnosed and frequently mismanaged, largely due to the overlapping presentation with primary forms and limited awareness of targeted diagnostic algorithms. Recent advances in understanding the dysregulation of the renin-angiotensin-aldosterone system, the identification of structural and functional renal vascular defects, and the characterization of hereditary and metabolic contributors have refined our comprehension of this condition. Modern imaging technologies, biomarker panels, and omics methodologies are enabling earlier detection and refined phenotyping, yet the integration of these tools into routine clinical pathways remains inconsistent. This growing body of evidence emphasizes the need for cohesive research linking molecular mechanisms to patient-tailored interventions. This Research Topic aims to advance the understanding of secondary hypertension by unifying basic, translational, and clinical research perspectives. The primary objective is to consolidate mechanistic insights and translate them into precision management strategies that enhance diagnostic accuracy, therapeutic efficacy, and long-term cardiovascular outcomes. This includes addressing fundamental questions about the interplay between genetic predisposition and environmental triggers, assessing novel imaging and biomarker technologies for their diagnostic power, and defining evidence-based treatment pathways tailored to individual etiologies. The overarching goal is to promote a paradigm shift from empirical therapy toward mechanism-driven, patient-specific management and, where possible, curative intervention. To gather further insights into the evolving landscape of secondary hypertension and its management, we welcome articles addressing, but not limited to, the following themes:o Pathophysiological mechanisms of primary aldosteronism and updates on subtype differentiation and therapeutic outcomeso Renovascular hypertension including fibromuscular dysplasia and atherosclerotic renal artery stenosiso Endocrine and metabolic drivers such as pheochromocytoma, Cushing’s syndrome, thyroid disorders, and obesity-related hypertensiono Genetic and monogenic forms of hypertension and their clinical implicationso Biomarkers and multi-omics approaches for risk stratification and diagnostic refinemento Non-invasive imaging and interventional innovations for early identification and treatment planningo Precision therapeutic strategies integrating pharmacological and procedural modalitieso Studies evaluating access disparities and multidisciplinary management modelsWe particularly encourage work that bridges mechanistic discoveries with clinical translation, fostering multidisciplinary collaboration across endocrinology, nephrology, cardiology, radiology, and genetics to deliver patient-centered advancements in the management of secondary hypertension.
