The Diagnostic Difficulties of Immune Mediated Neuropathies – Volume II
The field of neuroimmunology has made significant strides in understanding the heterogeneity and complexity of immune mediated neuropathies over the past decade. Despite diagnostic criteria such as the EFNS/PNS guidelines for chronic inflammatory demyelinating polyneuropathy (CIDP) helping to reduce misdiagnosis rates, a subset of patients continue to present with overlapping or atypical features that blur traditional diagnostic boundaries. Diseases such as CANOMAD, POEMS syndrome, and neurolymphomatosis remain challenging both to identify and to differentiate from CIDP, representing true diagnostic mimics that complicate patient management. Recent discoveries in autoantibodies targeting nodal and paranodal structures, as well as refinements in understanding disease specific pathomechanisms, have reshaped classification frameworks for autoimmune nodopathies and related disorders. Yet, clinical translation of these findings remains incomplete, and the search for robust biomarkers and imaging correlates continues. This second volume builds on the success of the first Research Topic, expanding focus toward emerging diagnostic approaches, innovative molecular markers, and evolving therapeutic strategies. It aims to synthesize evidence from electrophysiology, neuroimaging, serology, and pathology to refine differential diagnoses across the full spectrum of immune mediated neuropathies. By integrating clinical observations with laboratory and translational data, this collection seeks to bridge the gap between mechanistic research and clinical application, ultimately promoting more precise classification and individualized treatment. To gather further insights across this evolving field, we welcome articles that define the current boundaries and advance the understanding of immune mediated neuropathies through novel methodologies and interdisciplinary collaboration. Contributions may include original research, reviews, brief reports, and clinical perspectives. We invite submissions addressing, but not limited to, the following themes:• Novel diagnostic and electrophysiological techniques for differentiating immune mediated neuropathy subtypes• Autoantibody mediated mechanisms and their implications for updated disease classification• Discovery and validation of biological and imaging biomarkers for CIDP, MMN, and nodopathies• Advances in nerve ultrasound, MRI, and pathology correlation• Emerging therapeutic targets and personalized management strategies for difficult to treat neuropathies• Differential diagnosis and “true mimics” of CIDP and related conditions• Clinical and translational studies linking pathomechanisms with prognosis and treatment responseBy continuing this Research Topic series, Volume II will serve as an essential forum to consolidate ongoing advances in diagnosis, classification, and management of immune mediated neuropathies, driving forward precision medicine in this challenging neurological domain.
