The Role of GLP1 Receptor Agonists in Patients With and Without Diabetes and Metabolic Disease, Focusing on Renal Disease Prevention

The landscape of renal disease management is undergoing a significant transformation, particularly with the advent of novel therapeutic agents originally developed for metabolic disorders. Glucagon-like peptide-1 (GLP-1) receptor agonists have emerged as a cornerstone in the treatment of type 2 diabetes and obesity. However, recent clinical evidence has increasingly highlighted their profound renoprotective effects, which extend beyond glycemic control and weight loss. These agents have demonstrated the ability to mitigate renal inflammation, oxidative stress, and fibrosis, suggesting a direct organ-protective mechanism. As the global burden of chronic kidney disease (CKD) continues to rise, understanding the pleiotropic effects of GLP-1 receptor agonists is critical. Their potential to delay disease progression offers a promising new avenue for therapeutic intervention in patients with metabolic syndrome, as well as in those with non-diabetic kidney pathologies. This Research Topic aims to comprehensively evaluate the role of GLP-1 receptor agonists in the prevention and management of renal disease across diverse patient populations. We seek to gather cutting-edge research that elucidates the mechanisms underlying the renoprotective effects of these agents, both in the presence and absence of diabetes. A key objective is to explore the clinical utility of newer and emerging biomarkers in identifying patients who would benefit most from early intervention with GLP-1 receptor agonists. Furthermore, this collection intends to investigate the synergistic potential of combining GLP-1 receptor agonists with other established nephroprotective therapies, such as Sodium-Glucose Cotransporter-2 (SGLT2) inhibitors, to optimize treatment strategies and improve clinical outcomes in CKD.We welcome the submission of Original Research, Reviews, Mini-Reviews, Brief Research Report, FAIR² Data, General Commentary, Hypothesis and Theory, Methods, Opinion, Perspective, Systematic Review, Technology and Code that address the therapeutic potential of GLP-1 receptor agonists in renal pharmacology. Submissions should focus on, but are not limited to, the following themes:• The identification and validation of newer/emerging biomarkers to predict the efficacy of GLP-1 receptor agonists in halting renal disease progression through early intervention.• The pharmacological mechanisms and clinical outcomes of GLP-1 receptor agonists in patients with concurrent diabetes and chronic kidney disease (CKD).• The rationale, efficacy, and safety profile of utilizing GLP-1 receptor agonists in the management of non-diabetic CKD.• The clinical and mechanistic exploration of combination therapy involving SGLT2 inhibitors and GLP-1 receptor agonists in CKD patients, both with and without diabetes.• Preclinical models elucidating the direct molecular pathways through which GLP-1 receptor agonists exert their renoprotective effects.
