Thrombo-inflammation, vascular remodeling, and endovascular strategies in aortic dissection, aneurysm, embolism, and peripheral artery disease
The field of vascular medicine and endovascular therapy is witnessing an intense focus on the interplay between thrombo-inflammation and maladaptive vascular remodeling across diverse acute and chronic vascular diseases, including aortic dissection, aortic aneurysm, pulmonary embolism, and lower extremity artery disease. Despite their anatomical and pathophysiological differences, these conditions often share convergent mechanisms—namely, endothelial dysfunction, cross-talk between immune and coagulation pathways, extracellular matrix remodeling, and biomechanical stress. These shared processes drive vessel wall instability, promote thrombus formation or propagation, and ultimately result in vascular occlusion or catastrophic events such as rupture or tissue ischemia. Recent studies have underscored the importance of immune-coagulation interactions, the role of matrix metalloproteinases, and the significance of smooth muscle cell phenotypic switching, while also revealing the inadequacies in translating these mechanistic findings into actionable clinical practices. This Research Topic aims to bridge mechanistic understanding and clinical management by fostering studies that elucidate these shared pathways and translate insights into improved risk stratification, decision-making, and patient outcomes. By encouraging research centered on thrombo-inflammation, ECM biology, and the integration of technology-driven tools such as multimodal imaging, molecular biomarkers, and AI-powered models, the goal is to unify disparate disease models under common biological themes. Specific objectives include identifying actionable biomarkers or imaging markers to predict disease instability, optimizing patient selection and procedural timing for endovascular and hybrid interventions, and harmonizing outcome metrics across diseases to facilitate comparative and translational research. Ultimately, the aim is to inform real-world acute care decisions, procedural strategies, and long-term surveillance that are responsive to the underlying mechanisms of disease progression and thrombotic risk. The scope of this Research Topic encompasses mechanistic, translational, and clinical studies directly addressing thrombo-inflammation, vascular remodeling, and acute endovascular or hybrid care in aortic dissection, aortic aneurysm, pulmonary embolism, or lower extremity artery disease. Research focused solely on coronary artery disease, pulmonary vascular disease unrelated to acute PE, or purely technical device development without clinical or mechanistic insights falls outside the intended boundaries. We welcome articles addressing, but not limited to, the following themes:Immune–coagulation interactions and cellular mediators in thrombo-inflammatory responsesECM remodeling, matrix proteases, and smooth muscle cell plasticity in vascular instabilityAcute endovascular and hybrid intervention strategies: patient selection, timing, technique, and complication managementRisk stratification and phenotyping using advanced imaging, omics, and artificial intelligence with clinically meaningful endpointsMethodological comparisons and translational approaches spanning two or more vascular diseases, including harmonized biomarkers and outcome definitionsClinical trials, registries, and translational models linking mechanism to relevant outcomes (e.g., mortality, rupture, reintervention, limb events, recurrent VTE, and quality of life)Appendix:Article types considered include original research, clinical trials, translational studies, real-world evidence, registries, comparative and harmonization studies, and mechanistically informative case series (provided they yield practice-changing insights). Purely technical device papers, case reports lacking mechanistic insights, and studies unrelated to the four specified diseases are generally out of scope.
