Transcatheter Valve Interventions: Imaging, Patient Selection, Outcomes, and Complications in TAVR and TMVR
Transcatheter valve interventions have rapidly expanded from a therapy for select high risk patients to a mainstream option across a wider risk spectrum, with growing use in complex anatomies and diverse clinical settings. As transcatheter aortic valve replacement and transcatheter mitral valve repair or replacement mature, attention has shifted from procedural feasibility to optimizing lifetime management, durability, and patient centered outcomes. Advances in multimodality imaging, device design, and procedural techniques have improved safety and efficacy, yet important gaps remain in standardized patient selection, risk prediction, and the prevention and management of complications. This Research Topic aims to address persistent variability in real world practice and outcomes by highlighting evidence and innovation that refine pre procedural planning, procedural execution, and post procedural surveillance. Contributions should clarify how imaging and clinical phenotyping guide device choice and access strategy, how outcomes should be measured beyond traditional endpoints, and how complications can be anticipated, detected early, and treated using reproducible algorithms. The goal is to support consistent decision making, improve procedural success, and advance long term quality of care for patients undergoing transcatheter aortic or mitral valve interventions. We welcome original research, brief reports, systematic reviews, meta analyses, methods papers, and clinically focused perspectives. The scope includes, but is not limited to, the following areas:• Imaging for sizing, procedural guidance, and follow up, including computed tomography, echocardiography, cardiovascular magnetic resonance, fusion imaging, and AI enabled workflows;• Patient selection and risk stratification, including anatomy, frailty, comorbidities, lifetime management planning, and heart team pathways;• Outcomes and durability, including hemodynamics, valve thrombosis, structural valve deterioration, reintervention, and quality of life;• Complications and mitigation strategies, including paravalvular leak, conduction disturbances, coronary obstruction, stroke, vascular injury, left ventricular outflow tract obstruction, residual mitral regurgitation, and hemolysis;• Special populations and challenging scenarios, including bicuspid aortic valves, valve in valve procedures, severe mitral annular calcification, tricuspid valve disease, and combined or staged interventions;• Transcatheter valve platforms and bioprosthesis engineering, including tissue source and processing, leaflet and frame materials, anti calcification and preservation strategies, mechanisms of degeneration, and links to thrombosis risk and long term durability;• Lifetime valve strategy across technologies, including how prosthesis type and material properties shape durability, anticoagulation needs, surveillance, and reintervention planning, with surgical mechanical and surgical tissue valves discussed when relevant as clinical comparators.
