Understanding and Rehabilitating Pathological Fatigue across Acquired Brain Injuries, Neurological and Endocrine Conditions

Fatigue is a common, disabling, and insufficiently understood symptom across rehabilitation-related conditions such as neurological conditions (including traumatic brain injury) and endocrine conditions. Pathological fatigue can substantially affect daily functioning, participation, rehabilitation outcomes, and quality of life, yet it remains under-recognized and inconsistently assessed in clinical practice. Lack of standardized terminology, diagnostic criteria, symptom presentation, and measurement approaches continue to complicate comparisons across neurological conditions (including traumatic brain injury) and endocrine conditions, and evidence-based treatment and rehabilitation strategies remain limited. Investigation is needed to clarify shared and condition-specific mechanisms, improve assessment, and translate emerging knowledge into practical guidance for clinical care. This Research Topic aims to advance understanding of pathological fatigue across neurological conditions (including traumatic brain injury) and endocrine conditions, with particular emphasis on its implications for rehabilitation and everyday functioning. It will consider how pathological fatigue develops, how it can be reliably identified and measured, and how it influences patient-reported outcomes, participation, and quality of life. The Research Topic also seeks to examine the effectiveness and applicability of rehabilitation and treatment approaches, while identifying gaps in healthcare guidance and service provision. By bringing together mechanistic, assessment-focused, and clinically oriented research, it aims to support more consistent recognition, management, and rehabilitation of pathological fatigue across diagnostic groups. To gather further insights into pathological fatigue in neurological conditions (including traumatic brain injury) and endocrine conditions, this Research Topic welcomes articles addressing, but not limited to, the following themes:• Neural and physiological mechanisms of pathological fatigue in neurological conditions (including traumatic brain injury) and endocrine conditions• Assessment and measurement tools for pathological fatigue• Rehabilitation and treatment approaches• Cross-diagnostic comparisons of shared and distinctive pathological fatigue patterns• Patient-reported outcomes and the everyday functional impact of brain fatigue• The effects of pathological fatigue on rehabilitation participation and outcomes• Gaps in current healthcare guidance and service provision for fatigue managementWe welcome Original Research, Reviews, Mini Reviews, Perspectives, and Case Reports.
