Updates in Diagnosis and Treatment of Thymic Diseases
Thymic diseases, particularly thymic epithelial tumors (TETs), represent a complex and evolving field within mediastinal oncology. The anterior mediastinum can harbor a spectrum of thymic pathologies, ranging from indolent thymomas to more aggressive thymic carcinomas. While significant strides have been made in imaging modalities, surgical techniques, and therapeutic protocols, considerable gaps persist in optimizing diagnosis, standardizing treatment, and improving patient outcomes. The ongoing debate over surgical approaches, criteria for resectability, and the integration of novel imaging such as PET/CT in clinical decision-making highlights the necessity for a nuanced and unified understanding. Recent studies have demonstrated that advances in imaging, such as PET/CT, can enhance staging accuracy and better inform surgical planning. Furthermore, the role of extended thymectomy, especially in conditions like myasthenia gravis (MG), continues to be refined with new data emerging on patient selection and outcome prediction. Current research also compares various surgical approaches, including cervical, transsternal, and hemi/clamshell thoracotomy, to determine their impact on morbidity and oncological efficacy. Despite these advances, there remains a lack of consensus on optimal management pathways, and variation in clinical practice persists internationally. This Research Topic aims to bring together contemporary research and expert reviews detailing progress in the diagnosis, staging, and treatment of thymic diseases. Specific aims include clarifying the diagnostic criteria for thymic masses, evaluating the role of advanced imaging in preoperative assessment, and presenting up-to-date evidence on surgical and medical treatment options. It also seeks to illuminate how different surgical techniques influence outcomes, with a particular focus on integrating new innovations and evidence-based protocols into routine care. By consolidating multidisciplinary insights, this collection aspires to address clinical uncertainties and improve standards of care for patients with thymic pathologies. The scope of this Research Topic encompasses original research, reviews, and clinical perspectives that define, challenge, or expand current paradigms in the field of thymic disease management. Submissions may focus exclusively on either diagnostic advances or therapeutic innovation, or provide comprehensive analysis across the continuum of care. To gather further insights in thymic disease diagnosis and management, we welcome articles addressing, but not limited to, the following themes:• Evolving classification and histopathology of thymic epithelial tumors;• Comparative effectiveness of imaging modalities, including PET/CT, in staging and surgical planning;• Strategies for assessing and predicting surgical resectability;• Surgical approaches: cervical, transsternal, hemi/clamshell thoracotomy and their respective outcomes;• Extended thymectomy in the treatment of myasthenia gravis and other comorbidities;• Innovations in medical management and perioperative care;• Long-term follow-up and outcome measures for patients with thymic diseases;• Intrapleural chemotherapy and other locoregional pleural therapies for pleural dissemination/recurrence of thymic epithelial tumors. If relevant, please include original research articles, reviews, clinical case reports, and expert perspectives as part of your submission to this Research Topic.
